1. When sending samples, please provide two original copies of the Sample Inspection Entrustment Form including the authenticity information of the submitted sample with the official seal or identity information of the client). Make sure the handwriting is correct, clear and complete. As for special circumstances, please contact the business personnel. Please send the form along with the sample. If you have further questions, feel free to contact us.
2. For vulnerable and valuable samples, please specify the storage conditions in the form.
3. If samples for law enforcement or arbitration are sent by the judicial and administrative organs, introduction, a letter of attorney, sampling form and other valid documents should be provided.
4. If subsamples, subsidies, technical information are expected to be recalled, clientele should clearly state in the Sample Inspection Entrustment Form .
5. Inspection fee is charged according to relevant regulations. Fee should be paid before inspection. Under special circumstances, fees should be settled before receiving the inspection report, otherwise the center has the right to refuse to provide inspection report. Under normal circumstances, only one original report is issued.
6. The invoice is claimed with Sample Inspection Entrustment Form .
